Copyright Removal Request

At Forms Montana, we prioritize the protection of copyright and respect the rights of copyright owners. If you believe that your copyright has been infringed upon by material available on our website, we are committed to taking appropriate action under the Digital Millennium Copyright Act (DMCA) and other applicable intellectual property laws. We are dedicated to addressing each claim of copyright infringement with due diligence and promptness.

Submitting a Copyright Removal Request

To file a copyright removal request, please ensure that your notification is complete and includes the following information:

  • A physical or electronic signature of the copyright owner or a person authorized to act on their behalf.
  • Identification of the copyrighted work claimed to have been infringed.
  • Identification of the material that is claimed to be infringing or to be the subject of infringing activity and that is to be removed or access to which is to be disabled, and information reasonably sufficient to permit us to locate the material.
  • Your contact information, including your address, telephone number, and an email address.
  • A statement by you that you have a good faith belief that use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law.
  • A statement that the information in the notification is accurate, and under penalty of perjury, that you are authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.

Please send your copyright removal request to dmca@formsmontana.com. Once we receive a valid complaint, our response includes removing or disabling access to the allegedly infringing material.

Counter-Notification

If you believe that your content was removed in error, you may send a counter-notification. Your counter-notification must include:

  • Your physical or electronic signature.
  • Identification of the material that has been removed or to which access has been disabled and the location at which the material appeared before it was removed or access to it was disabled.
  • A statement under penalty of perjury that you believe in good faith that the material was removed or disabled as a result of mistake or misidentification of the material.
  • Your name, address, and telephone number, and a statement that you consent to the jurisdiction of the Federal District Court for the judicial district in which the address is located, or if your address is outside of the United States, for any judicial district in which Forms Montana may be found, and that you will accept service of process from the person who provided notification of the alleged infringement.

Counter-notifications should be sent to our designated copyright agent at dmca@formsmontana.com. Upon receiving a valid counter-notification, Forms Montana will follow the DMCA's procedures to reinstate the material in question.

Repeat Infringer Policy

In accordance with the DMCA and other applicable laws, Forms Montana has adopted a policy of terminating, in appropriate circumstances and at Forms Montana's sole discretion, users who are deemed to be repeat infringers. We also reserve the right to limit access to the site and/or terminate the accounts of any users who infringe any intellectual property rights of others, whether or not there is any repeat infringement.
